CA Position Purpose: Under supervision of the Program Manager, the
Intake Coordinator facilitates patient admissions to the program
and assists in gathering, organizing and reviewing information from
new clients to determine eligibility needs and next steps in the
service process. Ensure that all necessary documentation is
complete and accurate and facilitate smooth communication between
clients and internal department. Program Population: The Pathway
Home Housing Project is a HFH funded program, operated by JWCH. The
Team consist of a cohesive team of MSW, case managers, site
monitors and other professionals who collaborate to provide housing
and critical services to People Experiencing Homelessness (PEH) and
those who have previously experienced homelessness. The
clients/residents may come with behavioral and/or substance abuse
issues. The program utilizes the Harm Reduction Model and we “do
whatever it takes” to encourage success of the client. Principal
Responsibilities: In collaboration with on site HFH staff, accepts
patient referrals from HFH Pathway Homes which may include
encampment places, hospitals and other unhoused areas. Collects
intake data and prepares admission package for new clients Obtains
all appropriate consents and authorizations for patient
participation in the program and for staff to communicate
effectively with other service providers/agencies in order to
assist patients with linkages to services and resources, benefits
acquisition, coordination of services, patient advocacy, etc.
Submits intake admission packet to HFH staff and arranges for a
assessment within 24 hours of patient’s arrival Maintains
information and tracking of all intakes, providing intake/admission
reports on a weekly basis to the Program Director Assist patients
with obtaining income and/or establishing benefits, including
assisting with completion and submission of applications, arranging
appointments and coordinating transportation as required for
applications for health insurance benefits (e.g. Medi-Cal,
Medicare, etc.), disability benefits (e.g. Supplemental Security
Income (SSI), Supplemental Security Disability Income (SSDI), etc.,
and other sources of financial assistance, (e.g. Unemployment,
General Relief (GR), etc. Provides advocacy on behalf of patients,
as appropriate Adhere to all organizational safety policies,
procedures, and regulatory requirements to ensure a safe
environment for patients, staff, and visitors. Participate in
required safety and compliance trainings and apply learned
practices in daily work activities. Identify, report, and help
mitigate unsafe conditions, incidents, or hazards in the workplace.
Use equipment, tools, and supplies safely and responsibly in
accordance with organizational and regulatory standards. Maintain
awareness of infection control, occupational health, and emergency
preparedness procedures appropriate to the role. Documents
pertinent patient information, following established policy and
procedures Documents accurately in the DHS electronic medical
record, all referrals made, appointments scheduled, and patient
progress toward obtaining benefits Documents all data clearly and
thoroughly– using name and title, date and time Follows all DHS
requirements to gain access and utilize the electronic medical
record Maintains confidentiality at all times. Discusses any
patient information only with appropriate personnel when related to
the care being provided, Maintains confidentiality for all
documents that contain patient identifier information, Observes and
respects confidentiality of information in regards to fellow
employees. Responsibility and accountability for adherence to
organizational /department standards/policy. Observes department
guidelines concerning absences and reports absences in accordance
with departmental procedures, Self-identifies potentially unsafe
systems/processes/situations and takes initiative to report to the
supervisor, Observes department guidelines to conduct personal
business during breaks and lunch hour, Projects a positive
professional image and adheres to organizational dress code.
Participates in organizational programs/committees as assigned
Participates in assigned staff and other meetings, including
quality improvement, case conferences and in-service meetings,
Participates in safety programs, which may include assignment to
emergency response team, Performs other duties and tasks as may be
required or assigned. Team Work and Interpersonal Skills

Requirements: High School diploma or equivalency Bachelor’s degree
in related field or equivalent experience preferred. Two years’
experience in a health care setting providing intake management or
similar duties Willing and able to work well with low income,
homeless individuals, with disabilities and special needs Bilingual
English/Spanish preferred *All JWCH, Wesley Health Centers
