Job Description:
We are hiring a Hospice Liaison, Sales Representative for our
Santa Barbara branch. If you have a background in medical or
service sales and have an interest in helping families, then please

salary range for this position at commencement of employment is
expected to be between $80,000 - $95,000; however, pay offered may
vary depending on multiple individualized factors, including
job-related knowledge, licensure, skills, and experience. We also
have a no-caps based commission plus a $3,000 sign-on bonus.

request. JOB SUMMARY: Specialist /Sales Representative is a
professional representative of the Agency who functions as a health
care educator and intermediary between the Agency and hospitals,
facilities, health care professionals and the community. The
purpose of this role is to facilitate an uncomplicated transition
of patients to home care, hospice and caregiver services consistent
with the mission and goals of the Agency. DUTIES AND
RESPONSIBILITIES Responsible for identifying appropriate
accounts/patients through review and consultation with health care
professionals to assist in securing appropriate services and
follow-up support Helps to achieve and maintain continuity of
patient care by planning and exchanging information with other
professionals within the hospital, long-term care facilities,
physician offices, medical groups, case management offices and
others in the community Develops and maintains relationships with,
and identifies needs of community organizations, utilization review
nurses, discharge planners, case managers, physicians, hospitals,
nursing facilities, medical groups, and other health care
professionals who have an opportunity to utilize Agency services
Acts as a liaison between the Agency and the
physician/facility/hospital with regard to patient care and
education when necessary Maintains a level of knowledge of Agency
services to present to community health care providers and payor
sources Attends and participates in rehabilitation, discharge
planning and patient care meetings as requested Provides and/or
coordinates agency sponsored educational opportunities and
presentations within the community Provides appropriate education
and marketing materials to targeted and active accounts in order to
effectively promote utilization of Agency’s services Participates
in sales meetings to ensure compliance with operational practices
and appraisals of pertinent community related events Consults with
Coordinators and Directors of Patient Care Services to ensure
correct utilization of clinical services Promotes positive working
relationships with referral sources through effective communication
and problem-solving strategies to help facilitate appropriate
service delivery Assists Marketing Department with ideas for
appropriate educational materials. Maintains adequate supplies of
marketing materials for their territory Responsible for achieving
monthly goals as assigned in the individual Annual Sales Commission
plan on a consistent basis Prioritizes, organizes and manages
workload according to the needs of clients and the goals of the
Agency Acknowledge patient rights including privacy and
confidentiality Consistently display a professional appearance and
conduct in all settings. Interpersonal and interagency
relationships are to be positive and professional at all times
Assumes responsibility and displays reliability for the completion
of all tasks, duties, communications and actions Maintains
professional growth and development through networking, membership
in professional organizations and continuing education Available to
attend networking and sponsored events after business hours and on
occasional weekends, in order to promote Agency visibility in the
community Participates in performance improvement activities Abides
by Agency policy and procedures Accepts other duties as assigned
EDUCATION, EXPERIENCE AND TRAINING High School Diploma is required;
Associate’s Degree or higher preferred At least 2 years of client
service and sale experience, health care industry experience is
preferred Proven ability to achieve sales goals Must possess
excellent organizational skills and be detail oriented Must
demonstrate strong written and verbal communication skills and be
able to interact effectively with health care professionals,
patients, peers and management
